This certificate is issued to certify the change of name or surname by the applicants i.e after marriage, renewal of passport etc. Applicant needs to bring both old and new passport.
Download the Miscellaneous Services Form (new link for misc form) and submit along with:

  • Self attested copy of old passport (first and last page)
  • Self attested copy of latest passport (first and last page);
  • Self attested copy of visa/work permit/residence permit;
  • Proof of Residential address in South Africa. This can be a copy of any one of the following:
    • South African Driving license.
    • Water or landline telephone bill displaying applicant’s address
    • House Lease Agreement
    • State Identification Card
  • Any other document as specified by the Consular Officer;

Note: Please carry originals of all the supporting documents to be presented to the Consular Officer (photocopies of which are to be attached with the application).
