How to Apply: Download the Miscellaneous Services Form http://passport.gov.in/nri and submit along with:

  1. Application form can be downloaded from our website: (a link of new form to be provided) (Miscellaneous Services Application form)
  2. Copy of the self-attested parent/s (residing abroad/South Africa) passports first two and last pages, which contains personal particulars of the passport holder
  3. Copy of passport page containing visa/work permit/permanent residence status of the parent/s
  4. One passport sized photograph to be pasted on the application)
  5. Proof of Residential address in South Africa
  6. Affidavit as per specimen provided in the section ‘FORM’
  7. Original passport/documents will be returned immediately after verification
  8. Any other document as specified by the Consular Officer

PROCESSING TIME: FIVE WORKING DAYS
